HOW TO START A LIFE INSURANCE DEATH CLAIM

We've provided the claim form here for you, along with a short checklist of documents that will need to be submitted to start the claims process.


Step 1

Call us at (800) 387-7876 (English) between 9 a.m. and 6 p.m. ET, so a Client Service Representative can assign a claim number and notify the claims department.
Step 2

Complete the claim form.
Step 3

Submit the claim form along with the following documents:


  • A certified death certificate issued by the Funeral Home or the Province
  • A copy of the deceased's Government issued identification

Once you've completed the forms and contacted a Client Service Representative, the form and your documentation should be submitted to:
